Plenty of practices went digital and just moved the frustration from a clipboard to a tablet. Whether digital intake actually helps comes down to a few decisions.
Pre-visit vs. waiting room
The biggest gain isn't the form, it's when it's filled out. Send the link a day or two before the appointment, and the patient completes it at home with their insurance card and medication bottles in front of them. That is patient readiness: every form on the visit done before the patient walks in.
A tablet in the waiting room is still paperwork, still a delay. Intake done in advance also gives staff time to flag missing information before the visit.
Don't replicate paper, improve on it
Reproducing a paper form screen-for-screen isn't an upgrade. Use what paper can't do:
Conditional logic means patients only see questions relevant to them. No allergies? Skip the allergy details section entirely. New patient? Full history. Returning patient? Just updates since last visit.
Structured fields replace blank lines. A medication field captures name, dosage, and frequency, so there's less cleanup.
Validation flags a short policy number or a missing field while the patient can still fix it.
Make it work on phones
Most patients fill intake on a phone. Pinch-zooming and tiny tap targets make it worse than paper. Use single-column, touch-friendly forms, and test on a real phone.
Reduce the "where's my insurance card" problem
Insurance is where intake stalls: nobody knows their policy number by heart. Card photo upload keeps patients from abandoning there: they snap the card instead of typing numbers.
Connect forms to patient records
If staff retype submissions into another system, you've only moved the data entry. Submissions should land on the patient record, so returning patients don't start from scratch, with webhooks or CSV exports for anything else.
Measure and improve
Digital is measurable: completion rates, drop-off points, problem fields. If many patients quit at the consent page, fix that page.
What to actually look for in an intake platform
Six questions matter:
- Can patients complete forms before the visit? (magic-link emails, shareable links)
- Does it work well on phones? (test it yourself)
- Can patients photograph their insurance card? (instead of typing policy numbers)
- Does it support conditional logic? (so patients only answer relevant questions)
- Can data flow into your systems automatically? (webhooks, exports)
- Is it HIPAA-compliant? (encryption, BAA, US-hosted)
Everything else is secondary.
