You Need the Information Before the Visit
Intake gives the provider medical history, medications, allergies, and insurance before the patient walks in. Without it, the visit turns into an information-gathering session instead of care.

A good intake packet also communicates your policies: fees, cancellations, what to expect. And it captures the patient's goals. Documented upfront, both sides have a shared reference if questions come up later.

Treatment can't begin without informed consent, and a signed, dated record protects you if there's ever a dispute. "The patient was informed of our policies and signed on this date" beats "we talked about it at some point."
