Does Formisoft include patient management?

Yes. Every patient gets a complete profile with:

• Demographics: name, DOB, gender, email, phone, address
• Insurance information: primary and secondary coverage
• Medical data: medications, allergies, conditions (ICD-10), emergency contacts
• Documents: uploaded insurance cards, IDs, referral letters, lab results
• History: all form submissions, appointments, and communications
• Patient tags and automatic system tags (Front Desk)

When Patient Mode is enabled on a form, submissions automatically create or update patient records with no manual data entry.
