CRAFFT Adolescent Substance Abuse Screening

The CRAFFT Adolescent Substance Abuse Screening form implements the CRAFFT 2.1 questionnaire, the most widely recommended screening tool for substance use risk in adolescents and young adults ages 12 to 21. The acronym CRAFFT represents six screening questions about substance-related experiences: riding in a Car driven by someone under the influence, using substances to Relax, using substances while Alone, Forgetting things while using substances, being told by Family or friends to cut down, and getting into Trouble while using substances.

The digital form begins with Part A, which asks about any use of alcohol, cannabis, tobacco, or other substances in the past 12 months. Patients who report any use proceed to Part B, the six CRAFFT questions. Each positive response scores one point, with a score of two or more indicating a need for further clinical assessment. The form also includes the Car question as a stand-alone safety screen, since riding with an impaired driver represents an immediate danger regardless of the patient's own substance use patterns.

This template is designed for pediatric practices, adolescent medicine clinics, school-based health centers, and any clinical setting serving teens and young adults. The CRAFFT is endorsed by the American Academy of Pediatrics for routine use during adolescent well visits and is included in Bright Futures screening guidelines.
