Tips for More Readable Patient Forms

  • Write at a 6th grade reading level. The AMA, NIH, and CMS all recommend this for patient materials.
  • Replace medical jargon with plain language. Say “high blood pressure” instead of “hypertension.”
  • Keep sentences under 20 words. One idea per sentence.
  • Use short, common words. “Use” instead of “utilize,” “help” instead of “facilitate.”
  • Write in active voice. “Sign this form” instead of “This form should be signed.”
  • Test your forms with real patients and ask if anything was confusing.
