How it works

  1. Build Your Assessment Form

    Create a form with radio or select fields for each question. Use a pre-built template (PHQ-9, GAD-7) or build your own assessment from scratch.

  2. Configure Scoring

    Open form settings and enable scoring. Formisoft auto-detects scorable fields. Assign point values to each answer option, or apply a one-click preset for standard assessments.

  3. Define Severity Ranges

    Set score ranges with labels and severity levels, for example, 0-4 Minimal, 5-9 Mild, 10-14 Moderate, 15-19 Moderately Severe, 20-27 Severe.

  4. Scores Appear Automatically

    When a patient submits the form, the score is computed server-side and shown on the submission detail page, in the submissions list, and in API/webhook payloads.
