Scored Assessments

Auto-score PHQ-9, GAD-7, and custom clinical questionnaires

Clinical assessments like the PHQ-9 (depression), GAD-7 (anxiety), AUDIT (alcohol), and dozens of other standardized questionnaires require scoring to be clinically useful. Formisoft computes scores automatically at submission time so your clinical team sees results instantly — no manual tallying, no spreadsheets, no errors.

Set up scoring in your form settings: Formisoft auto-detects scorable fields (radio buttons and dropdowns with options), lets you assign numeric values to each answer choice, and define severity ranges with labels. For common assessments, one-click presets configure PHQ-9 and GAD-7 scoring with standard clinical ranges.

Scores appear on every submission: a prominent score card shows the total, maximum possible, and severity classification. The submissions list also displays scores in a dedicated column so your team can triage at a glance. Scores are included in webhook payloads, the REST API, and PDF exports — making them available everywhere your data flows.

How it works

1
Step 1 of 4

Build Your Assessment Form

Create a form with radio or select fields for each question. Use a pre-built template (PHQ-9, GAD-7) or build your own assessment from scratch.

2
Step 2 of 4

Configure Scoring

Open form settings and enable scoring. Formisoft auto-detects scorable fields. Assign point values to each answer option, or apply a one-click preset for standard assessments.

3
Step 3 of 4

Define Severity Ranges

Set score ranges with labels and severity levels — for example, 0-4 Minimal, 5-9 Mild, 10-14 Moderate, 15-19 Moderately Severe, 20-27 Severe.

4
Step 4 of 4

Scores Appear Automatically

When a patient submits the form, the score is computed server-side and shown on the submission detail page, in the submissions list, and in API/webhook payloads.

Key features

Everything included with scored assessments, built into the platform.

One-Click Presets

Apply standard scoring for PHQ-9 (depression) and GAD-7 (anxiety) with a single click. Correct ranges and severity labels are configured automatically.

Custom Scoring

Assign any numeric value to any answer option on any radio or select field. Build scoring for custom assessments, satisfaction surveys, or risk evaluations.

Severity Classification

Define named ranges (Minimal, Mild, Moderate, Severe) with color-coded badges. Staff see the severity at a glance on every submission.

Score Card on Submissions

A prominent score card displays the total, maximum possible, and severity badge at the top of every submission detail page.

Scores in Submissions List

When scoring is enabled, the submissions table shows a score column so your team can triage assessments without opening each one.

Included in API & Webhooks

Scores are included in webhook payloads, the REST API, and PDF exports. Your external systems receive the computed score alongside submission data.

Server-Side Computation

Scores are computed server-side at submission time and stored immutably. They cannot be tampered with from the client.

Works with Templates

Pre-built screening templates (PHQ-9, GAD-7, AUDIT, and more) include scoring configuration. Create from template and scoring is ready to go.
