Formisoft offers two plans:

• Intake ($49.99/mo): unlimited HIPAA intake forms, AI form builder and PDF upload, 200+ templates, e-signatures and consent, insurance card capture, email magic links, automatic intake reminder emails 48 hours before the visit, and form analytics
• Front Desk ($149.99/mo): everything in Intake plus online booking and calendar, SMS and email appointment reminders, SMS and email patient inbox, online payments and copay collection, patient tags, readiness tracking for every appointment, webhooks, REST API, and EMR connectors
• Optional add-on: insurance verification on Front Desk, $0.50 per check, pay as you go with no monthly fee
• Enterprise: contact sales for SSO, SLAs, and dedicated support

Intake is limited to 1 seat. Front Desk includes 1 seat; additional team members are $19.99/mo each. Card payments carry a 5% processing fee per transaction. BAA included on every plan. There is no self-serve free trial; our team can grant a Front Desk trial on request.

Yes. No long-term contracts and no cancellation fees:

• Cancel from Settings > Billing > Manage Billing
• Data and access remain available until the end of your billing period
• Reactivate at any time

• SSO (Single Sign-On)
• Custom operational handoffs
• Dedicated support and SLAs
• Volume pricing for large organizations
• Priority feature requests

Each plan includes one team member. On Front Desk you can invite more:
