Treatment Outcome Survey

The Treatment Outcome Survey collects patient-reported outcome (PRO) data to measure how effectively a treatment or intervention has worked from the patient's own perspective. Unlike clinical measures alone, patient-reported outcomes capture the dimensions of health that matter most to patients: symptom relief, functional ability, quality of life, and overall well-being. This template provides a structured framework for collecting these outcomes at defined intervals after treatment, creating a longitudinal record that demonstrates treatment value and informs clinical decision-making.

The survey asks patients to compare their current status to their pre-treatment baseline across multiple domains. Patients rate the degree of symptom improvement, their ability to perform daily activities, any ongoing side effects or complications, and their overall satisfaction with the treatment outcome. A functional status section captures changes in mobility, energy level, sleep quality, and ability to work or participate in social activities. Patients also indicate whether they would choose the same treatment again and whether the results met their expectations.

This template serves orthopedic practices tracking surgical outcomes, pain management clinics measuring intervention effectiveness, oncology programs documenting quality of life during treatment, mental health practices assessing therapy progress, and any provider participating in outcomes-based reimbursement models. The collected data supports MIPS quality reporting, shared decision-making conversations with future patients, and continuous improvement of treatment protocols based on real-world patient outcomes.
