Formisoft sends a magic-link email with a combined form: standard intake fields plus telehealth-specific consent (technology requirements, privacy, recording policy).
Patient completes everything before the call
The patient fills out demographics, medical history, and signs consent on their phone or computer. Auto-save ensures nothing is lost.
Provider reviews and starts the session
The provider opens the submission, reviews intake data and signed consent, and starts the video session. Patient record is already created.
